NCS Live Auction Consignment Form

Instructions

  1. List each item or multi-item lot and check appropriate condition (HL = hairline; CR = crack; CP = chip; RP = repair). See auction rules and instructions for further information.
  2. Mark each item with stick-on dot (yellow or white) bearing your consignor code and item number (Mark cracks and chips with red dot).
  3. Fill out sales record card for each item.
  4. Send a copy of the completed form to Sally Stefferud prior to convention or submit it at convention check-in
  5. Bring form and cards to auction check-in.
